At Cori Manor Health Care, we understand that no one looks forward to entering a skilled nursing facility, even if it is only for a short term stay. The role of our Admissions Department is to assist families who need to find immediate placement for a loved one. Often, a Discharge Planner or Social Worker at a hospital informs a patient’s primary contact of the need for short term rehabilitation or long term care placement. Once this decision has been determined, families must quickly choose a facility.

We encourage families to tour and meet with our Admissions staff so they can see the facility first-hand and ask any questions they may have regarding this process. Once the family has chosen a facility, they inform the Discharge Planner or Social Worker of this decision. Next, the Discharge Planner or Social Worker will make an official referral to our facility. Our Social Worker or Nurse Manager, then, conducts and on-site assessment and notifies the referral source, the patient and the family of acceptance.

Admission to Cori Manor Health Care will be based on the type of care needed, the environment most appropriate for the Resident, and the availability of the accommodations.  We truly believe and insist that each Resident receives the best possible care at the least viable cost to the Resident in the most ethical manner, regardless of race, color, handicap, sex, creed, national origin, ancestry, sexual orientation, marital status or religion.

We accept inquiries and admissions 24 hours a day and 7-days per week including holidays. We also welcome on-site tours at all times. At Cori Manor Health Care we will always strive to make this challenging transition as smooth as possible for everyone involved.
